Booking Terms for Ultraderm Aesthetics

Welcome to the Ultraderm Aesthetics family! To ensure a seamless and enjoyable experience, please take note of our booking terms:

Booking Deposit:

A non-refundable deposit of $50 is required to secure your appointment. This deposit will be applied towards the total cost of your treatment.

Policies

Our UltraDerm Aesthetics policies will be provided at appointment.

Forms

Client forms will be emailed to the client once an appointment is scheduled.

Pre and Post-Care Instructions

At the time of scheduling your appointment, we will ensure that you are fully prepared by providing detailed pre-treatment instructions. Additionally, post-treatment procedures will be discussed to ensure optimal results and to guide you in following the necessary steps for recovery and aftercare.

Rescheduling:

We understand that scheduling conflicts may arise. You may reschedule your appointment up to 24 hours before the scheduled time without forfeiting your deposit.

Arrival Time:

For Laser and Microneedling Treatments: Please arrive at least 1 hour prior to your scheduled appointment to allow time for any necessary preparations.
For all other services: Please arrive at least 15 minutes before your scheduled appointment.

Consultation:

For certain treatments, a consultation may be required before scheduling the service. Please inquire with our staff to determine if a consultation is necessary for your chosen treatment.

Late Arrival:

We understand that unforeseen circumstances may arise. However, arriving late may result in a shortened treatment time to avoid inconveniencing other clients. In such cases, you will still be charged the full service cost. Arriving more than 10 min late may result in your appointment being canceled and the forfeiture of your booking deposit.

Payment:

Full payment is required at the time of service. We accept cash, credit cards, and other forms of electronic payment.

No-Call No-Show Policy:

Failure to show up for your appointment without prior notice will result in forfeiture of your booking deposit.

Health Considerations:

Please inform us of any health concerns, allergies, or medical conditions that may affect your treatment. This information is crucial for ensuring your safety and the effectiveness of the services provided.

Age Requirement:

Clients under the age of 18 must be accompanied by a parent or legal guardian, who must provide written consent for the services.

Child and Pet Policy:

At Ultraderm Aesthetics, we love kids and pets! However, their presence can pose safety concerns. Therefore, we kindly ask that children and pets do not accompany you to your appointment. We reserve the right to refuse service to anyone whose children are a safety risk to themselves or others. Please be aware that if you bring children to your appointment, Ultraderm Aesthetics does not assume responsibility for the care and/or supervision of your children at any time. Additionally, in adherence to health and safety regulations, pets are not permitted on our premises.

Photo Consent:

By booking an appointment, you grant Ultraderm Aesthetics the right to take photographs and/or videos of you. These images may be used for promotional purposes, including but not limited to social media, website content, and marketing materials.

If you do not wish for your photographs to be utilized for promotional purposes, please inform your provider prior to your service.

Cancellation / No-Show Policy:

Cancellations made within less than 24 hours, excluding weekends and holidays, are subject to a $50 fee and notices less than 24 hours, excluding weekend and holidays, or no-show appointments are subject to a $100 fee.

Please understand that a pattern of repeated no-show appointments may result in dismissal from the practice. Only calls (or voicemails) to the clinic are valid cancellations.

Appointments may not be cancelled via email or social media.
