A non-refundable deposit of $50 is required to secure your appointment. This deposit will be applied towards the total cost of your treatment.
Our UltraDerm Aesthetics policies will be provided at appointment.
Client forms will be emailed to the client once an appointment is scheduled.
At the time of scheduling your appointment, we will ensure that you are fully prepared by providing detailed pre-treatment instructions. Additionally, post-treatment procedures will be discussed to ensure optimal results and to guide you in following the necessary steps for recovery and aftercare.
We understand that scheduling conflicts may arise. You may reschedule your appointment up to 48 hours before the scheduled time without forfeiting your deposit.
We understand that unforeseen circumstances may arise. However, arriving late may result in a shortened treatment time to avoid inconveniencing other clients. In such cases, you will still be charged the full service cost. Arriving more than 10 min late may result in your appointment being canceled and the forfeiture of your booking deposit.
For certain treatments, a consultation may be required before scheduling the service. Please inquire with our staff to determine if a consultation is necessary for your chosen treatment.
Failure to show up for your appointment without prior notice will result in forfeiture of your booking deposit.
Full payment is required at the time of service. We accept cash, credit cards, and other forms of electronic payment.
Please inform us of any health concerns, allergies, or medical conditions that may affect your treatment. This information is crucial for ensuring your safety and the effectiveness of the services provided.
Clients under the age of 18 must be accompanied by a parent or legal guardian, who must provide written consent for the services.
At Ultraderm Aesthetics, we love kids and pets! However, their presence can pose safety concerns. Therefore, we kindly ask that children and pets do not accompany you to your appointment. We reserve the right to refuse service to anyone whose children are a safety risk to themselves or others. Please be aware that if you bring children to your appointment, Ultraderm Aesthetics does not assume responsibility for the care and/or supervision of your children at any time. Additionally, in adherence to health and safety regulations, pets are not permitted on our premises.
By booking an appointment, you grant Ultraderm Aesthetics the right to take photographs and/or videos of you. These images may be used for promotional purposes, including but not limited to social media, website content, and marketing materials.
If you do not wish for your photographs to be utilized for promotional purposes, please inform your provider prior to your service.
We kindly request a minimum of 48 hours' notice for any appointment cancellations. Your $50 booking deposit will be applied to your account and put towards a future service if your appointment is canceled within 48 hours of your scheduled appointment time. Failure to provide adequate notice may result in the forfeiture of your booking deposit.
